Speakers Stefan Boes, PhD and Sarah Mantwill, PhD from the university of Lucerne will discuss the Swiss Learning Health System.

Promoting and supporting uptake of evidence and evidence-informed decision-making in health-systems related policy and practice is a challenge. In Switzerland, the need to address this matter has been increasingly emphasized by different actors in the health system. In particular, the lack of comprehensive coordination efforts in the field of health services research, and subsequent knowledge translation activities, has been stressed. In response, the Swiss Learning Health System (SLHS) was established as a nationwide project in 2017, currently involving 10 academic partner institutions. One of the overarching objectives of the SLHS is to bridge research, policy, and practice by providing an infrastructure that supports learning cycles by: continuously identifying issues relevant to the Swiss health system, systemizing relevant evidence, presenting potential courses of action, and revising and reshaping responses. Key features of learning cycles in the SLHS include the development of policy/evidence briefs that serve as a basis for stakeholder dialogues with actors from research, policy and practice. Issues that are identified to be further pursued are monitored for potential implementation and eventually evaluated to inform new learning cycles and to support continuous learning within the system.

Dr. Boes and Dr. Mantwill will provide an overview of the SLHS and its key features, as well as its capacity building efforts to train young researchers in the field of learning health systems, and the development of a centralized metadata repository in support of creating a sufficient large evidence basis to support learning cycles in the Swiss health system. Further, they will discuss lessons learned from the past and the newest developments of the SLHS in light of a second funding phase supported by the Swiss government.

Techniques and Challenges for EHR Phenotyping

Electronic health records (EHR) contain a wealth of real world data that can be used for research purposes. However, extracting phenotype information from EHRs can be challenging. EHR phenotyping can be divided into two types: (1) Fast phenotyping which seeks to capture a broad swath of the medical phenome, and is often accomplished using coded EHR data (e.g. billing codes) and (2) slow phenotyping that seeks to achieve high precision and recall for a single phenotype, and often uses multiple EHR data types (e.g. medications, text, lab results). This talk will describe specific use-cases for both fast and slow phenotyping, and review challenges that are commonly encountered in creating research-grade EHR phenotypes.

The development, validation, and implementation of machine learning (ML) models for use in healthcare requires a strong understanding of clinical needs, analytical methods, and systems engineering. A deep dive into the development of a ML model for predicting return to work of patients experiencing occupational injuries will be used to anchor a broad discussion on engineering ML for medicine. This discussion will cover the development of a dynamic prediction model using workers compensation claims data. Additionally, we will briefly cover issues surrounding ML model task framing, validation, and implementation.

Erkin Ötleş is a Medical Scientist Training Program Fellow (MD-PhD student) at the University of Michigan. He has completed three years of medical school training and is currently a PhD candidate in the department of Industrial and Operations Engineering. His research lies at the intersection of computer science, industrial engineering, and medicine, centered on creating machine learning and artificial intelligence tools for patients, physicians, and health systems. Erkin’s dissertation work focuses on the development, implementation, and prospective usage of dynamic health outcome prediction models (e.g. early warning systems). He is co-advised by Dr. Brian Denton (Industrial and Operations Engineering) and Dr. Jenna Wiens (Computer Science and Engineering). Erkin has a professional background in health IT - having managed electronic health record development and healthcare data science teams. He holds a Master’s of Industrial Engineering from the University of Wisconsin. After completion of his MD-PhD training, Erkin plans on pursing medical residency training in emergency medicine.

Unlike structured data, unstructured data are often buried within free text clinical narratives that are difficult to analyze and interpret to derive useful insights. Free text cannot be easily categorized in the same way that a structured, numerical data point can, and unstructured data often have nuances that are not easily captured or represented in structured data.

This session will cover methods and techniques for interpreting and converting unstructured text into useful research data using two related, but distinct approaches: (1) Natural Language Processing (NLP), a specialized branch of AI focused on the interpretation and manipulation of human-generated spoken or written data; and (2) information retrieval, which often underlies many search engine technologies. This session will also highlight EMERSE, an open-source information retrieval tool that has been designed to help everyday users work with the free text documents (i.e., clinical notes) in medical records that is now being adopted by other academic medical centers.

Finally, attendees will hear directly from researchers about how they have used these methods and tools to enhance their research by accessing and harnessing the power of unstructured data.

Abstract:

In the Peixoto lab we use genomic approaches to understand gene expression and its epigenetic regulation in response to learning and sleep deprivation, and its alteration in autism spectrum disorders. This requires combining behavioral paradigms in mice, molecular biology and the analysis of high-throughput data in the brain in vivo. It also requires using the right data analysis tools to be able to capture the effect of learning or sleep in the context of an ever-active brain. In this talk we will discuss the effects of learning on chromatin accessibility and the effects of sleep loss in gene expression, with an emphasis on how data analysis influences our ability to detect novel and reproducible biology.

Lucia Peixoto received her bachelor’s degree in Biochemistry from the Universidad de la Republica in her native Uruguay in 2002. She subsequently earned her Ph.D. at The University of Pennsylvania under the mentorship of Dr. David S. Roos, using genomic and computational biology approaches to understand host-pathogen interactions. She completed her postdoctoral training in Neuroscience with Dr. Ted Abel at The University of Pennsylvania in 2015. During her fellowship, she was also a trainee at the Training Program in Neurodevelopmental disabilities at the Children’s Hospital of Philadelphia. As a trainee at CHOP, she completed a clinical internship at the Center for Autism Research under the supervision of Dr. Robert Schultz. She became an Assistant Professor at Washington State University in 2015 and has since been recognized with a K01 Early Career Faculty award from NIH/NINDS and a pilot award from the Simons Foundation Autism Research Initiative. She is also a member of the board of directors of the International Society of computational biology (ISCB) and cochair the Equity, Diversity and Inclusion committee of ISCB. Her lab uses behavior, electrophysiology, molecular biology and genomic approaches to understand how sleep and learning modulate transcription and how this may be altered in Autism Spectrum Disorders.

The session will describe the landscape history, current status, and future of federated health data networks that are used to support a Learning Health System. Dr. Brown will describe the creation, infrastructure, operation, and uses of several networks from the perspective of a network coordinating center. Dr. Harris will describe insights from participating in multiple networks as a network partner, including infrastructure, governance, and operational lessons learned.

Unstructured clinical data, such as clinical notes and reports, along with the computational infrastructure and tools, have seen an increasing demand from the research community in the last years, mostly fueled by recent advances in statistical and machine-learning approaches to data insight. We are meeting this demand with the Information Commons – a research data platform that hosts and provides direct access to de-identified data, advanced analytics tools, and computational environments for our research community. 

While we are realizing access to de-identified electronic health records, images, omics and biobank data, this session highlights the progress made to provide more than 110 million de-identified notes to the research community. We developed and operationalized a fully automatic de-identification algorithm and implemented EMERSE, a user-friendly tool for non-programmatic access and sophisticated textual searches on the de-identified clinical notes.   

As of December 2021 Our de-identification algorithm and our clinical notes are certified de-identified and are currently available for the UCSF researchers with IRD. The presentation covers the entire pipeline from data extraction to publication and data access focusing on the secured computational infrastructure. Furthermore, we discuss the rigorous evaluation techniques to ensure the quality of the deidentification process and the resulting data according to HIPAA and UCSF Security and Privacy protection requirements. Lastly, we showcase highlights from our research collaborations enabled by this new resource of machine-redacted, unstructured clinical notes linked with de-identified structured EHR data using EMERSE and their impact on the research community.
